These Terms of Service govern your use of revivallongevitywellness.com and any services offered by Revival Longevity & Wellness, a healthcare practice located in Winter Park, Florida. By accessing or using this Site, you confirm that you are at least 18 years of age.

4. Financial Policies

Revival Longevity & Wellness is a self-pay practice. We do not accept insurance as payment for our services. Patients are responsible for the full cost of services at the time of or prior to treatment. We accept cash, major credit cards, HSA funds, and FSA funds. Financing options may be available and will be discussed at your consultation.

5. Text Messaging (SMS) Terms

Program name and description. The Revival Longevity & Wellness Text Messaging Program sends customer care and marketing text messages to patients and prospective patients who opt in. Customer care messages include appointment reminders, scheduling updates and confirmations, and replies from our team to your questions. Marketing messages include special offers, referral rewards, event invitations, and news about our clinic and services.

How you opt in. You join the program by checking the optional text message consent box on the contact form on our website and providing your mobile number. Checking the box covers both customer care and marketing text messages. Consent to receive text messages is not a condition of purchasing any goods or services, and you can submit our forms without opting in. By opting in, you confirm that you are the account holder or an authorized user of the mobile number you provide.

Message frequency and rates. Message frequency varies based on your appointments, your conversations with our team, and our current offers. Message and data rates may apply. Contact your wireless carrier for details about your plan.

How to opt out. You can cancel at any time. Reply STOP to any message to opt out. You can also reply CANCEL, END, QUIT, or UNSUBSCRIBE. After you opt out, you will receive one message confirming that you have been unsubscribed, and you will receive no further text messages from us unless you opt in again. You may also opt out by telling us in any reasonable way, including by calling or emailing us.

Delivery. Carriers are not liable for delayed or undelivered messages.

Your health information and emergencies. To protect your privacy, our text messages do not include detailed medical information, and we ask that you not send sensitive health details by text. Text messages are not monitored for emergencies. If you are experiencing a medical emergency, call 911.

6. Governing Law

These Terms are governed by the laws of the State of Florida. Any disputes shall be resolved in the state or federal courts located in Orange County, Florida.
